NOTICE OF CONSUMER HEALTH RECORD ACCESS & COMPLAINT INFORMATION
This notice is posted in compliance with HB 4224 (Texas Health & Safety Code §181.102–§181.103; HB 4224)
This notice provides consumers with instructions on how to request access to health care records, contact applicable licensing or regulatory authorities, and file a consumer complaint, as required by Texas law.
Requesting Your Health Care Records You have the right to request access to your health care records maintained by this practice.
How to Submit a Request Health care records may be requested by phone, email, or physical mail
All requests require confirmation of identity before records can be released.
How Records May Be Provided Upon request and verification of identity, records may be provided by in-person physical pick-up, certified mail, or secure digital delivery through an individual patient portal.
Delivery of records by email requires written authorization from the patient or the patient’s legally authorized representative.
