Patient Forms

Once you arrive at Moody Health Center, our receptionist will welcome you to the clinic and have you complete our patient intake forms.  This paperwork provides us with necessary information including your health history and specifics about your condition.

These forms are also available to download and print in the New Patient Center so that you may complete them in advance and bring them to your appointment for convenience.

Consultation

Next, you will have a consultation to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for use to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

Examination

After the consultation confirms that yours is a chiropractic case and with your consent, an examination will be performed evaluating your posture and mobility as well as any necessary orthopedic and neurological testing to determine your diagnosis.

X-Ray Studies

Depending on your specific condition and circumstances, x-rays may be necessary to rule out and alert us as to any serious spinal conditions and assist in the establishment of your diagnosis as well as the development of your specific treatment recommendations,

Report of Findings

After reviewing your health history and completing the appropriate examinations, the next step will be a detailed report of all findings including confirmation regarding the appropriateness of chiropractic care in your case or any necessary referrals to other providers, clarification as to how the recommended treatment will help, how often and how long you will need to come in for care and what the treatment will cost. Any questions that you have will be answered at this time before proceeding with treatment.

Treatment

At this point, treatment can begin which may include spinal or extremity adjustments, physical modalities, and/or manual therapy techniques. Treatment is patient centered and the recommendations will take into consideration your specific goals.

Home Instructions

Prior to leaving, the doctor will give instructions for your care between visits.  This may include: ice or heat application, positions and activities to avoid, and home stretches/exercises.
